Scheduling Appointments
To schedule an appointment, please call our office between 8:00am to 6:00pm, Monday through Friday. At that time, we will ask you to explain the nature of your visit.
Canceling Appointments
If you must cancel or reschedule your appointment, we appreciate 2 business days notice. This notice will allow another patient to use your appointment time.
Certain in office procedures such as Prostate Biopsy, Ultrasounds and Urodynamics are subject to a cancellation fee. All surgical procedures are also subject to a cancellation fee if not cancelled within 5 business days.
Late Arrivals
We respect the time of all our patients. We strive to stay on schedule so that you do not have to wait. If you are delayed and arrive late for your appointment, every effort will be made to see you the same day. On occasion, you may be asked to wait or reschedule your appointment.
Emergencies
If you are our current patient (i.e. have been previously seen by one of our doctors in the office), there is a physician on-call 24 hours a day, 7 days a week to help you with an urgent medical problem. If your medical condition is life threatening please call 911.
If the emergency occurs during office hours, call the main number to get in touch with a physician.
If the emergency occurs after office hours, call the main number and you will be instructed on how to contact the physician on call. The physician on-call will return your call as quickly as possible. Please limit your calls to true emergencies. If you have Caller ID Block, you must disable it to receive a phone call from the on-call physician.
If your problem is not a medical emergency, please call during office hours. Prescription refill requests will be handled only during office hours.
Medical Records
You are entitled to copies of your medical records, for yourself or for another medical provider. After providing our office with the proper written authorization, the office will release your records. Download the medical release form here.
There is a fee for obtaining your medical records, please contact our office for more information.
Please allow 7 business days for us to prepare and mail/fax your medical records.
Prescription Refills
All routine prescription refills should be requested thru your Pharmacy. All requests for prescription refills are done either thru the E-prescribing system to our patient's pharmacy OR by Fax requests. NO routine prescription refills will be handled thru telephone requests.
If you are on a long-term medication, our physicians will typically prescribe a sufficient quantity to last until your next visit. PLEASE BE SURE TO REQUEST ALL REFILLS AT THE TIME OF YOUR APPOINTMENT. If you are running out of medicine, it is likely because you need to make a follow-up appointment. Please be sure to check if you are current with your scheduled appointments prior to requesting refills.
Telephone Consultations
Most health insurance benefits do not include telephone consultations as a covered health benefit. In order to receive a medical consultation, an appointment must be made.
Referrals
Most health plans require referrals for you to see a specialist.
If you are a new patient and have been referred to our office with a medical condition by another physician, you will be required to provide us with a referral. If you arrive at our office without a referral, you will be asked to reschedule your appointment for another day.
